Dental Insurance Information

To utilize your dental insurance benefits, we require immediate receipt of the insurance details requested on the Online Patient Forms.

NOTE: If insurance information is provided at the appointment time, we cannot verify your benefits, and you will be responsible for the full payment of services provided that day. Please provide details of all dental insurance, including any for a spouse or a medical health plan with a dental rider. Verify insurance with your spouse and employer’s HR department.

Dental Insurance and Payment for Services

We understand that the financial aspect of dental services can be a concern. We provide a detailed cost estimate for each procedure, showing what is covered by insurance and your out-of-pocket portion. Please remember this is an estimate and your insurance reimbursement may vary. You are responsible for the full balance on your account, regardless of insurance coverage. We accept various payment methods, including cash, personal checks, debit cards, major credit cards, and dental insurance plan payments.

Patient Forms

New patient forms will be emailed and must be completed at least two business days before your appointment. If you cannot complete them in advance, please arrive 30 minutes early, and we will provide an iPad to complete the forms. Incomplete forms may reduce your appointment time and limit the procedures we can complete.

Privacy Policy

Cancellation Policy: A $75 fee applies to missed appointments or changes made without at least 2 business days’ notice (Monday-Friday)
